Chicago logged 112,057 traffic crashes in 2024. Each one produced a police report that feeds straight into the insurance investigation. But the department doesn’t mail those documents out. A Chicago police crash report has to be requested, and this guide covers the mechanics of doing it.
What Is a Chicago Traffic Crash Report (SR1050) and why is it needed?
Illinois law approves exactly one form for reporting crash data to IDOT: the SR 1050. The responding officer fills it out at the scene. Inside you’ll find a hand-drawn collision diagram, a written narrative of what happened, policy numbers for both drivers, injury codes on the KABCO scale, and any citations issued. Under 625 ILCS 5/11-408, police have 10 days after the investigation to forward the finished traffic crash report to IDOT.
The document does little for you in court. Illinois Rule of Evidence 803(8) pulls police accident reports out from under the public records exception, so juries generally never see them. For an insurance carrier, though, it’s the central piece of paper. The adjuster opens your accident report first and reads the officer’s diagram to decide where fault sits.
What You Need Before You Request the Report
You can’t pull a Chicago Police Department crash report with a last name alone. The search asks for three things:
- The RD number CPD assigns to every incident
- The date of the crash
- The driver’s first and last name
An RD number runs two letters followed by six digits. It’s printed on the Driver Information Exchange card the officer hands out at the scene. Lost the card? Records staff will look the number up at (312) 745-5130.
How to Request Your Chicago Crash Report
Online (Fastest)
The portal at crash.chicagopolice.org holds reports from 2016 through today. Enter the RD number, the crash date and the driver’s name, pay by card, download the file. It won’t be there the morning after your wreck. The state takes up to 24 hours just to assign a control number, and the document itself loads into the system a few business days later. Anything older than 2016, or filled out on paper, exists only at the records window.
In Person
Records Customer Service Section sits at 3510 South Michigan Avenue, first floor, Room 1043. The window runs Monday through Friday, 8:00 a.m. to 1:30 p.m., closed on public holidays. City pages list conflicting hours, so call before you drive over.
By Mail
Mail requests go to Unit 163 at 3510 S. Michigan Ave., Chicago, IL 60653. Include your name and address, the location of the crash, the RD number, a self-addressed stamped envelope, and a check or money order payable to Department of Revenue, City of Chicago. The city publishes no turnaround time for this route.
How Much Does a Chicago Crash Report Cost?
| Report type | How requested | Cost |
|---|---|---|
| Traffic crash report | Online portal | $6.00 |
| Amended report | Online portal | $1.50 |
| Traffic crash report | In person or by mail | $5.00 |
| Case report copy | In person or by mail | $0.50 |
| Accident reconstruction report | In person or by mail | $20.00 |
Ordering online is the fastest option and covers reports from 2016 to the present. That $6.00 breaks down into $5.00 for the document and $1.00 for payment processing, a split CPD spells out on the information card it gives drivers. An amended report costs less than a full one. You order it when the officer got a policy number wrong or drew the direction of travel backward.
How a Chicago Car Accident Lawyer Can Help?
Your Chicago police crash report is where the case starts. It hands your attorney details you never caught or never had time to write down at the scene: witness names, the other driver’s carrier, the officer’s note on what caused the collision. Then comes the work that happens outside the report. Errors get challenged through correction requests, statements get checked against camera footage, and the insurer’s convenient read of the police diagram gets met with a reconstruction of your own.
The clock is shorter than most people assume. Illinois gives you two years from the crash date to file an injury suit (735 ILCS 5/13-202), and a CTA bus or train in the mix cuts that to one year (70 ILCS 3605/41).
